Typically, ovulation will cause a few key changes in your body that can be tracked:

  1. Cervical Mucus (CM): Right before ovulation, your CM will begin to change. It gets thinner, more slippery, and sometimes stretchy (think egg whites). This is a sign that your body is preparing itself to allow sperm to swim to the egg.

  2. Basal Body Temperature (BBT): After ovulation happens, your body’s internal temperature will rise and stay elevated for the rest of your cycle, a sign that a hormone called progesterone has been released to support a potential pregnancy.

  3. LH Surge: An LH surge is a spike in the luteinizing hormone (LH) that happens just before ovulation. Ovulation predictor kits (OPKs) can be used to test for this surge, but women with PCOS may see false positives, making this tool trickier to rely on.

How to Read Your BBT Chart: A sustained temperature rise of at least 3 days in a row is a strong indicator you’ve ovulated. Your temperature should stay elevated until you either get your period (if pregnancy hasn’t occurred) or a pregnancy test confirms otherwise. This is the sign that you want to look for!

Look for a Sustained Temperature Rise

While BBT is great for confirming ovulation after it’s happened, tracking cervical mucus (CM) can help you pinpoint your fertile window and trust what you’re seeing with your temperatures. In the days leading up to ovulation, your cervical mucus will change in texture and quantity.

With PCOS, your mucus patterns may be a little different, but you can still look for the typical signs:

  • At Ovulation: You’ll notice that your CM becomes clear, slippery, and stretchy, resembling egg whites. You may feel a slippery sensation and see no mucus. 

By tracking both your BBT and CM, you get a fuller picture of what your cycle’s indicators look like. The combination of these two signs can help you pinpoint when ovulation is most likely happening for you, even if you feel like your cycle length is too unpredictable.

Be Patient And Don’t Stress!

If you have PCOS, you might be testing for ovulation with OPKs and not seeing a clear positive result every month. That’s totally normal! In fact, many people with PCOS can experience false positive results or no surge at all, even when they do ovulate. So don’t get discouraged if you miss a surge, or don’t get a definitive result on an OPK.

If you haven’t experienced a sustained rise in temperature along with a change in your cervical mucus, you probably haven’t ovulated yet. If it’s past day 21, then you know that your ovulation is delayed. Keep charting and tracking your data so that when you do ovulate, you can be confident in your fertility signs.
